Gm/Bb 	- Gm chord with Bb as bass tone
Bb(/D) 	- you can play Bb major chord with D bass (but its up to you)
| 	- bar separator, anyway listen to the song for correct timing
In solo:
4/12	- slide from 4th fret to 12th
e	- eight note

Its simple and beautiful song. I like it the most from X&Y album.

You can play solo on the H string only, but I think this is 
closer to original and its fuller this way.

So tune up and play. Go falzeto singing - if you can :)
